Congrats on making the Slideshare homepage - a great preso deserves it! 2 thoughts.
1. I lived through the Denver baggage debacle. Bags were actually destroyed. Rumor was they tried to use a realtime version of Windows NT to operate this...need I say more?
2. Maybe the Mother of All Software Disasters? Apollo 13. Heard Fred Haise (Bill Paxton played him in the movie) speak about the problem: NASA had 2 databases - one for testing on the ground and one for space flight. The switch settings were different; someone used the wrong database; and bang! the switch blew when the oxygen tanks were stirred.
